MHD EndPoints

MHD transactions are now supported, as profiled by CH EPR FHIR, and available through the /pmp/fhir and /pmp/mhd endPoints:

Provide Document Bundle (ITI-65) /pmp/fhir/Bundle MHD equivalent to ITI-41.
Find Document References (ITI-67) /pmp/fhir/DocumentReference MHD equivalent to ITI-18.
Retrieve Document (ITI-68) /pmp/mhd/iti68 MHD equivalent to ITI-43.
Update Document Metadata (CH:MHD-1) /pmp/fhir/DocumentReference MHD equivalent to ITI-57.
CH:PHARM-5 /pmp/fhir/DocumentReference/<operation> MHD equivalent to CH:PHARM-1. The endpoint changes depending on the CH:PHARM-5 operation, see the transaction page for details.
